At Faith Home Care Practitioners PLLC , we are focused on providing medical care services to patients who are homebound or have difficulty getting in to see their primary care providers.

Our providers share a combined experience of over 20 years in family practice and gerontology. They are experienced in diagnosing, treating, prescribing, and managing acute and chronic care conditions, as well as providing primary care services with the highest level of customer satisfaction. With a variety of services to choose from, we're sure you'll be happy working with us.

Julie Calderon,

DNP, MSN, ARNP, FNP-C

Dr. Julie Calderon is a family nurse practitioner licensed in Washington state, and has been in practice since 2013. She founded Faith Home Care Practitioners in 2015 when she observed how difficult it was for her older adult patients to access care due to their impairments.

She has a B.S. in Healthcare Administration from California State University, B.S. Nursing from Ferris State University, Masters in Nursing from Michigan State University, and a Doctorate in Nursing from Missouri State University.

She has a passion for chronic disease management and caring for the geriatric population. She has been a healthcare entrepreneur with her spouse, Emilio, since 2007. She is interested in developing and implementing new care delivery models that improve patient access and quality of care.

Outside of work, she enjoys spending time with her family, including her two teenagers, Olivia and Emilio Jr. She also enjoys traveling, binge watching movies and K-dramas, and listening to K-pop music with her daughter.

 

Derick Potter, MSN, ARNP, FNP-C

Derick Potter is a board certified family practice nurse practitioner licensed in Washington State.  He has been in practice since 2015 with experience in family practice, long term care, and memory care.

He has a Bachelor of science in Biology from St. Martin’s University, Bachelor of Science in Nursing from Desales University, and a Masters in Nursing from Maryville University. 

His model of care includes a personalized care approach to maintain health with a focus on chronic disease management, preventative care, and spiritual and emotional health.  

Outside of work he enjoys spending time with his wife and children. He also enjoys snowboarding, golfing, fishing, hiking, and anything on the BBQ.

 

Cattleya Nery,

MSN, ARNP, AGPCNP-BC

Cattleya Nery is a compassionate, board certified Adult Gerontology Primary Care Nurse Practitioner dedicated to enhancing the quality of life for older adults.

Cattleya holds a Master of Science in Nursing with a specialization in Adult Gerontology Primary Care from Walden University and a Bachelor of Nursing from Ferris State University.

Her practice emphasizes holistic, patient-centered care, focusing on managing chronic conditions, preventing disease, and promoting healthy aging. Her approach is characterized by empathy, patience, and a genuine commitment to improving her patients' quality of life.

Outside of her professional duties, Cattleya enjoys running and staying active, Audible books, and spending time with her husband and 3 daughters.

 

Karla Rotante, RPH

Karla Rotante is a clinical pharmacist. She completed her Bachelor of Science in Pharmacy majoring in Clinical Pharmacy from the University of the Immaculate Conception in Davao City, Philippines.  She has been in practice for 4 years, most of which as a Hospital Pharmacist. She does her best to instill genuine care for her patients, provide the necessary help, and improve her quality of work. We are excited to have her on our team!

Outside the workspace, Karla is a full-time mother as well who enjoys bringing Samantha wherever she goes, and her partner wants her to cook pasta more often.

Emilio Calderon

Emilio Calderon has served as FHCP’s practice manager since 2015. He has over 20 years of customer service experience and 13 years of healthcare entrepreneurship. He is bilingual in both English and Spanish. He previously owned a non-emergency medical transportation company that transported patients to and from their medical appointments. He shares an enthusiasm for improving healthcare delivery to patients with limitations.

He is most passionate about the Seattle Seahawks, the Los Angeles Lakers, and Real Madrid Football. He is also an avid shoe collector and a “Sneakerhead”.
